The Transition from School to Active Life (TEVA) is a collaborative planning process designed to help students prepare for life after high school. It brings together the student, their family, the school team, professionals, and community partners to develop a plan based on the student’s strengths, interests, needs, and future goals.

This resource provides an overview of the TEVA process and the different areas that can be considered when planning for adulthood, including education and training, employment, personal autonomy, housing, transportation, social networks, leisure, and community participation.

A useful resource for parents, professionals, and service providers supporting adolescents and young adults as they prepare for the transition from school to adult life.
